Our Responsibilities

Elixia Hydration & Wellness is committed to protecting health information in accordance with applicable privacy and security requirements. We are expected to maintain privacy, provide notice of privacy practices, and follow the terms of the notice currently in effect.

Permitted Uses and Disclosures

Protected health information may be used or disclosed for treatment, payment, healthcare operations, appointment reminders, quality improvement, legal compliance, public health reporting, and other purposes permitted or required by law.

Patient Rights

• Request access to or copies of certain health records.
• Request corrections to health information.
• Request limits on certain uses or disclosures.
• Request confidential communications.
• Receive an accounting of certain disclosures when applicable.

Authorizations

Uses or disclosures not otherwise permitted by law may require written authorization. Patients may revoke an authorization in writing, except to the extent action has already been taken in reliance on it.
